Should the mailing address, and principal (physical) address, and company names be the same on the operating authority (MC number) applications as it is in the US DOT number record

Yes. When completing the OP-1 application for operating authority (MC number), the information must be consistent, including the company name, doing business as name, principal (physical) address, mailing address, and etc. Ensure that the operating authority (MC number) information matches the US DOT number record information.
