How can I reactivate my USDOT number?

You may need to reactivate a DOT number for several reasons:

  • Failure to complete the biennial update
  • Failure to complete the New Entrant Program

To reactivate a USDOT number, you will need to mail or fax a completed and updated MCS-150 form. You can only submit it online if:

  • You are reactivating a US DOT number that was deactivated due to failure to complete the Biennial Update: You must check the box “Biennial Update or Changes” in the “Reason for Filing” section
  • You are reapplying after a New Entrant Revocation: You must check the box “Reapplication (after Revocation of New Entrant)” in the “Reason for Filing” section

If you are a Mexico-domiciled carrier or your number has been deactivated for reasons other than not filing your Biennial Update, then you will need to mail or fax a completed and updated MCS-150 form. You must check the box “Biennial Update or Changes” in the “Reason for Filing” section.

You can submit the paper form in one of these ways:

  • By email: You can upload and submit the updated MCS-150 via our web form (you will receive a tracking number)
  • By mail: You can send the updated MCS-150 form to:
