What We Do

The OMI addresses health care problems to monitor and improve the quality of care provided by VHA.


Listening To You 

If you, or a Veteran you know, has a problem with VHA medical care, phone, email, or write us.

Our Hotline Number is 800-634-4782.  We take calls from 7:00 a.m. to 5:00 p.m. ET on weekdays. Our voice mail operates 24/7.

What We Need to Know to Help You

To access the correct medical records, we need your name, your SSN (the last four is enough), your date of birth, and the Medical Center you attend.

To reach you, we need your phone number and your preferred address.

Please do not detail your problem on voice mail: we will be in touch with you to get the details, usually within the next business day.
