Have you received an official survey in the mail about your visit to a provider within the Bassett Army Community Hospital (BACH) system? Don't throw these surveys away! They are a chance to provide BACH with anonymous feedback which the command staff monitors carefully.

APLSS, Army Provider Level Satisfaction Survey, is a survey that is administered by the Office of the U.S. Army Surgeon General. This survey focuses on issues ranging from how the provider communicated with the patient during the visit, access to care, cleanliness of the facility, and courtesy of the staff. Patients are randomly mailed a letter asking them to complete a brief questionnaire regarding their care at BACH. The survey consists of 24 questions. There are three different ways a patient can complete the questionnaire. The first is by calling the toll-free number and utilizing a voice response system. The second method is completing the written survey and mailing in the provided pre-addressed and postage paid envelope. The third, and preferred method, is an Internet based survey where the patients can log in with the username and a password provided in the letter sent to them about the survey. The results of this survey are confidential and provide immediate feedback to the leadership of BACH as well as the Office of the U.S. Army Surgeon General.

All comments and concerns are always a top priority for every member of your BACH team. We continuously strive to provide the best healthcare for our patients. Completing the survey gives you, the patient, the opportunity to have your voice heard by BACH leadership. We use the survey results to identify areas for performance improvement. You also have the ability to formally recognize people or services that performed exceptionally. Additionally, funds for your hospital are also generated based on your responses and overall satisfaction ratings.

So next time you get an APLSS letter in the mail, please take the time to complete the survey. The responses that you give the BACH team help all our patients receive the best possible care.
