The Pediatric Care Clinic encompasses two Patient Centered Medical Homes; the Yellow Home and the Green Home. 

All children over 10 years of age need to present their Military ID at their appointment. If they do not have an ID or do not have an ID with them, they will be asked to get an authorization for care form from Medical Records.

Physicals

It is recommended that everyone have a yearly physical. Therefore, schedule physicals during the appropriate birth month. DOD schools recommend that your child have a physical before starting school, entering 6th grade, and/or enrolling in high school physical education classes. A current Sports Physical (dated within one year) is required before practicing or participating in any sport. Physical exams, documented on the Kentucky High School Athletic Association (KHSAA) form, must be signed and dated by the health care provider. Civilian Schools require a physical at initial entry into the school system, sixth grade and for anyone participating in sports. They also require vision and dental exams before school and then annually if indicated. If your child has had a physical within the past 9 months and needs a sports form completed, drop off the form at the front desk with your information completed. The staff will get the form completed and signed and you can pick it up after 72 hours.

Referrals

Unless you are in an emergency situation (where death, loss of limb or sight) is involved, all referrals MUST be pre-authorized. This pre-authorization process takes about two (2) weeks to complete can be tricky, but we are here to help. All new referrals will require an evaluation from your primary care provider. Please call central appointments to schedule your visit. If it is an on-going referral that needs to be renewed, we may be able to renew it without another visit. Please refer to the list below to identify the RN that is on the team with your provider.

For all referrals that are appointed to the Military Treatment Facility, you can book the appointment by calling central appointments at 800-493-9602 or by calling the specialty clinic directly.

However, some specialty referrals require a nurse to book the first appointment. If you are instructed to have a nurse call to book the appointment please refer to the list below to locate the appropriate point of contact.  

Refills

Many medications can be refilled by calling the Registered Nurses on your provider’s team listed at the number above. This process may take a maximum 72 hours to complete, as we must pull the records, review the last visit comments and speak with your provider; but every effort will be made to return your call within a few hours.

We apologize, but due to infection control we are currently unable to keep toys in the clinic for children to play with.

LATE ARRIVALS: The policy of the Pediatric Clinic for late arrivals is as follows: if you are 15 minutes late, you may be asked to reschedule your appointment. This is not meant as a punishment those who are late, it is out of respect for patients who arrive early or on time.
