Special Circumstances
Here are the necessary procedures to follow to request mail-in identification card (note that this only applies to Bldg 253, Soldier Service Center located on Pony Ave):
Verification of circumstances may be required. The following are examples of persons who may be unable to report in person to obtain an ID card:
- Persons who are physically handicapped
- Persons who have no means of transportation
- Persons who are hospitalized or sick
- Persons who live far from a military facility
Applicants must provide an up-close 8” x 10” or 5” x 7” portrait that is notarized on the back, along with physical characteristics (eye and hair color, weight and height and if applicant is an organ donor or not). A notary must verify the applicant’s signature on a written statement concerning his or her identity. Instances of medical impairment, a statement from the attending physician is acceptable. Information must be sent by certified mail.
Along with the above documentation the following is required:
- Copy of front and back of current ID card
- Copy of secondary identification
Note: Mail-in requests are processed to the nearest ID card issuing facility.
