Inbound Soldiers
Congratulations on your assignment to Fort Lee!
If you haven’t already done so, complete the Sponsorship Program Counseling and Information Sheet, DA Form 5434 (DA Form 5434.pdf) and send it to the Fort Lee Sponsorship Program Manager. If you are a part of the Army Career Tracker (ACT) Total Army Sponsorship Program (TASP), complete the DA Form 5434 in the ACT TASP at https://actnow.army.mil. Your request will be forwarded to your gaining unit/agency and the designated sponsor will contact you by email or via the ACT system with information about your new unit/agency.
You may also request youth sponsorship and information for your middle school and high school children.
Go online to www.leemwr.com, then click on Child and Youth Services.
To request Youth Sponsorship, select the Youth Services tab, and then click on the Youth Sponsorship link at the bottom of the page.
Complete the form, save it and then email it to the
Youth Sponsorship Manager. To find information about area schools
and successful transitions, go to the Child and Youth Services page and select the Local Schools tab.

Outbound Soldiers
One of the most important pre-move actions you can take is to request a sponsor. Please complete the Sponsorship Program Counseling and Information Sheet, DA Form 5434 (DA Form 5434.pdf) and see your unit's sponsorship coordinator. Sponsorship Survey
An important part of completing the in- or out-processing checklist is the online Total Army Sponsorship Program Survey. Click on the appropriate link below to complete your survey.
If in Army Career Tracker (ACT) Total Army Sponsorship Program (TASP), complete the Sponsorship in-processing survey in ACT by accessing the Sponsorship tab and the Forms subtab. The survey will be saved in ACT and show as completed.

Sponsorship Training
All Soldiers are eligible to be assigned as a Sponsor to assist inbound and outbound Soldiers. For training and certification, access the following link to the Sponsorship Application and Training (eSAT). In addition, sponsorship training is available through ALMS and ATRRS for Soldiers assigned as sponsors. The course is "Total Army Sponsorship Training."
