Below are the forms required for you to complete your Command Sponsorship request:

• Out of Country Forms (currently not in Korea)
• • DA Form 4787 Reassignment: Provided to you by your reassignments work center
  • DA Form 5888 Family Member Deployment Screening Sheet: Provided to you by your reassignments work center.
  • DD 2792 Exceptional Family Medical Summary: Provided by screening agency if required.
  • DD 2792-1 Special Education Early Intervention Summary: Provided by screening agency if required. Sex Offender Declaration Memorandum: Example
  • NOTE: 2792 and/or 2792-1 with IEP are only required if DA Form 5888 is annotated “Consideration Warranted.

  Requests being submitted while the Soldier is still inbound to Korea , and must be submitted by the Soldiers servicing MPD to Korea MPD no more than 180 days from or 30 days to the Soldiers report date to usarmy.yongsan.8-army.list.csp-korea@mail.mil

  • DA Form 5888 Family Member Deployment Screening Sheet: Initiated by the Soldier and authenticated by the unit S1. Get Form
  • DD 2792 Exceptional Family Medical Summary: Provided by screening agency if required.
  • DD 2792-1 Special Education Early Intervention Summary w/IEP: Provided by screening agency if required.
  • Request for In Country Command Sponsorship Memo
  • Sex Offender Declaration Memorandum: Example
  • Current Leave and Earning Statement (LES)
  • Travel Voucher Advice of Payment (AOP) from Soldier PCS to Korea
  • NOTE: 2792 and/or 2792-1 with IEP are only required if DA Form 5888 is annotated “Consideration Warranted.

• Already submitted a Command Sponsorship Packet and want to check the status?
  After receiving your CSP tracking number from your local MPD, go to: https://www.us.army.mil/suite/page/671823
  After logging in through AKO, use the number provided by MPD to track the status of your request.
