Implementing RCA2: Maine Medical Center & Cedars Sinai Medical Center

Description

This presentation will focus on the experience of Maine Medical Center and Cedars Sinai Medical Center in implementing the recommendations of NPSF’s recent report, RCA2: Improving Root Cause Analyses and Actions to Prevent Harm. Maine Medical Center, a 637 licensed-bed teaching hospital located in Portland, Maine, will discuss their adverse event assimilation and notification processes, as well as the inclusion of involved clinicians in the RCA2 team and event time line preparation. Cedars Sinai Medical Center, a non-profit, tertiary 958-bed hospital and multi-specialty academic health science center located in Los Angeles, California, will share their process for choosing the correct events to undergo RCA2, and their innovative use of their Simulation Center for completing RCAs. These organizations will be joined by Dr. Jeffrey Cooper, a patient safety expert who is dedicated to the use of simulation in health care as a means to improve the process of education and training and to avoid risk to patients.

Learning Objectives:

  • Gain a better understanding of the Root Cause Analyses and Actions (RCA2) process and how it can be effectively implemented across a variety of organizations
  • Learn about tools and best practices for implementing RCA2 from organizations at different places in their RCA2 journeys
  • Gain greater insight into important aspects of the RCA2 process, including:
    • The use of risk-based prioritization systems
    • Effectively engaging leadership
    • Setting up and maintaining RCA2 teams
    • Utilizing the tools in the report to determine, and follow up on, effective actions
