The Reserve Health Readiness Program (RHRP) provides the following services to members of the U.S. Coast Guard Reserve.

Periodic Health Assessment
The Periodic Health Assessment (PHA) is a screening tool used by the armed forces to evaluate the individual medical readiness of their Service members. It can be conducted alone or can be combined with other individual medical readiness needs (e.g., dental exam and immunizations). Completed annually, the PHA consists of the following components for the Service member:
- A self-reported health status.
- Measurement and documentation of vitals (height, weight, BP)
- Vision screening
- Review of current medical conditions with healthcare provider.
- Focused exam of identified conditions (as required).
- Cardiovascular Screening Program Services (as required).
- Recommendations for improvement of identified health conditions.
- Behavioral health screen.
Immunizations
Immunizations are offered in-clinic or through group events. At group events:
- The RHRP vendor provides all staffing, equipment and supplies
- Minimum requirement for group event is 60 total immunizations (not 60 total service members)
- Provide the space for group events in a temperature controlled environment with access to tables and chairs
- Enter the data into the appropriate medical databases after the event
Dental Services
The dental examination is a screening tool utilized by the United States Armed Forces to evaluate the individual dental readiness of their Service members.
Completed annually, the dental examination consists of the following components:
- Exam Indication: All Service members without a current dental exam within the past 9 months will usually receive a new dental exam.
- Health History: Service member completes health history.
- Bitewing X-Rays: Taken with every dental exam.
- Blood pressure: Will be recorded for every dental exam.
- Panoramic X-ray (pano): No time requirement, to be taken if no pano is in the dental record, if significant change has occurred since the last one, or if deploying.
- Dental Examination: Each Service member will be seen by a licensed dentist.
- Review of Health History and appropriate x-ray(s).
- Periodontal Screening and Recording (PSR).
- Charting of all dental disease. Classification designated.
- Original exam documents entered into Service member’s record.
- Exam QA: Dental exams and ancillary documents are shipped to RHRP and reviewed. After review completed documents are mailed back to Service Member’s (SM) Medical Unit
- Digital Storage: All copies of records are digitized and stored at the vendor location
Service Delivery:
All dental services available individually or as a group event. For a group event:
- Minimum requirement is 50 dental exams per Group Event
- The RHRP provides all staffing, equipment, and supplies needed to perform all dental services.
- Your unit is responsible for providing space for group events to include:
- Temperature-controlled environment
- Access to electrical outlets
- Access to tables and chairs
Post-Deployment Health Readiness Assessment
The Post-Deployment Health Reassessment (PDHRA) is a health screen designed to protect and evaluate the health of those returning from combat.
- Targeted at 3-6 months after return from deployment
- Provides education, screening, assessment, and access to care for a wide variety of questions and concerns that Service members may have about their health.
- Povided through the toll-free call center or on-site at a group event.
The PDHRA is offered individually through the call center, and the RHRP contractor enters all data into the PDHRA within ASIMS in real time.
- SM completes SM portion of the DD Form 2900 and calls the PDHRA call center at 1-888-734-7299
- A licensed Physician Assistant (PA) or Nurse Practitioner (NP) reviews the document with you and determines if you need further evaluation or treatment.
- You'll be referred for treatment or further evaluation if deemed necessary
Physical Exams
The physical exam is used to evaluate the individual medical readiness of their Service members.
- Physical exams are only offered in-clinic.
The exam consists of the following:
- A self-reported health history
- Measurement and documentation of vitals (height, weight, BP, vision, etc. )
- Review of current medical conditions with healthcare provider
- Identification and examination of potential health risks
- Collection and analysis of lab specimens (as required)
- Recommendations for improvement of identified health conditions and to minimize potential health risks
Mental Health Assessments
The Mental Health Assessments (DHA4 and DHA5) are a series of deployment health screenings designed to identify mental health concerns, including post-traumatic stress disorder (PTSD) and other behavioral health conditions that may require referral for additional care and treatment. These screenings are done in order to ensure individual and unit readiness. The DHA 4 and 5 are required by all Service members deploying for 30 days or more in support of any contingency operation.
The DHA 4 is designed to be completed between 181 days and 18 months (545 days) after return from deployment, and the DHA 5 between 18 months (546 days) and 30 months (910 days) after return from deployment. The DHA 4 and 5 assessments are the final screenings of the 5 part DHA assessment cycle that includes the pre-deployment health assessment, post-deployment health assessment, and post-deployment health reassessment.
Labs
RHRP will draw blood samples from Service members to meet readiness and pre-deployment requirements. The U.S. Coast Guard Reserve is responsible for processing, labeling, and shipment of all labs.

Annual services include the flu vaccine, dental exam and the PHA, to include vision screening. Audio screening may be annual depending on your branch of the military (please contact your command).

What constitutes a no-show?
A:
A no-show is when a Service member misses an appointment or does not call RHRP at least 24 hours prior to the appointment to cancel or reschedule the appointment.

Is there a cost for RHRP services?

There is no cost to you for authorized services. RHRP services are paid for by your Service Component. If you have specific questions about costs or charges, please contact your medical readiness coordinator.

How do I schedule and submit an event?

USAR and ARNG have the option of entering an event voucher in the Army’s Automated Voucher System (AVS). All other branches should contact the IMR Department at RHRP at 1-800-666-2833, ext 3508 for assistance.

How do I get a service expedited?
A:
In very rare instances (e.g. very short notice deployment to a disaster site) you can get services expedited.
- For an individual appointment, the command’s IMR representative should call Customer Service at 1-800-666-2833.
- For a group event, the command’s IMR representative should call their event coordinator or 1-800-666-2833, ext. 3555.

What are the cancellation and no show policies?

- Decreasing services requested with 14 and 6 calendar days of the event date will result in a Cancellation Fee.
- Decreasing services requested within 5 calendar days of the event date will result in a No-Show Fee.
- Failure to meet GE minimums will also result in a No-Show Fee.

How do I start my PHA?
A:
For USAR only, and only for an in-clinic appointment: you need to start the process on your AKO site. Complete the Service member portion and call Customer Service at 1-888-697-4299 to schedule the face-to-face appointment. If you are having problems accessing your AKO site, call the number above and complete the Service member portion over the phone.
If you are getting your PHA at a group event, you should not complete the AKO portion of the PHA as this information will be obtained at the event.
All other branches contact your command.

What is a CAE?
A:
A Comprehensive Audio Evaluation (CAE) is more than just an audiogram and is used to determine the hearing portion of the PULHES when a possible loss of hearing has been identified. The CAE includes a bone-density screening and can also include a SPeech Recognition In Noise Test (SPRINT) if needed.

What do I need to bring to my clinic appointment?
A:
RHRP will send (by Federal Express) a package of paperwork and/or supplies to you prior to your appointment. Please review and complete all necessary paperwork before the scheduled appointment and bring all materials with you. Also bring any relevant medical documentation (i.e., MRB, profiles, etc.) and your prescription eyeglasses if attending a PHA.

What happens if I refuse services at an appointment or a group event?
A:
If you have documentation to show you’ve had the service, you should make a copy and give it to the provider. If you just don’t want the service or are unsure if you need the service, you should contact your Command’s IMR representative for guidance before refusing the service, as there may be repercussions with your command for doing so.
