What to expect when you arrive
You will be greeted by our front desk staff. For your first visit or a new injury, you will complete an intake questionnaire and verify third-party insurance.

For your follow-up appointment or treatment, simply announce your arrival to the front desk have a seat until called by your provider.

Physical Medicine & Rehabilitation Service
Sports Rehabilitation
  • Sports Medicine specialists treat injuries with both surgical and non-surgical techniques, as well as with aggressive, proactive rehabilitation.
  • Restoring physical condition and function for knee and ankle dysfunction.
Knee School
  • Learn anatomy, ergonomics, and causes of pain. Strengthening and stretching exercises will be demonstrated

Physical Therapy Service
As our patient, you are a partner in your rehabilitation. Physical therapists and technicians provide you with expert knowledge, resources and guidance in the treatment and prevention of recurrent injuries.

Terms we use in your care...
Physical Therapy Evaluation – your first visit to a physical therapist.

Physical Therapy Follow-Up – subsequent appointments with a physical therapist to monitor your progress and reassess your treatment plan and goals.

Treatment Plan – schedule and detailed guidelines for your treatment.

Home Exercise Program (HEP) – treatment guidelines for your home program.

Range of Motion – measure of a joint's mobility; may be active, passive or active-assisted.
