How to Make a Request
To obtain accommodations from CAP, a CAP Accommodation Request Form must be completed and submitted. There are three options available within the form:
- I know exactly what solutions I need. Select this option if you already know which assistive technology you would like to request.
- Recommend solutions for my needs. Select this option if you would like to answer a series of questions, which will provide a list of technology that commonly assists individuals who experience similar limitations.
- I want CAP to contact me. Select this option if you would like to provide information in the request form and have a CAP staff member contact you to determine the best method to conduct a needs assessment.
We recommend browsing some of the most frequently requested assistive technology on the CAP website prior to submitting a request.
If you are located in the DC Metro area, you can contact the CAP Technology & Evaluation Center (CAPTEC) located in the Pentagon to schedule an in-person assessment. CAPTEC has an array of technology for demonstration and gives you hands-on experience with potential solutions during your visit.
