SECNAV Revises & Reissues OPNAV Personally Identifiable Information (PII) Breach Reporting Forms

Initial and after-action PII breach reporting forms OPNAV 5211/13 and 5211/14 have been revised and reissued, with the same titles, as SECNAV forms 5211/1 and 5211/2, respectively. DON policy for PII breach reporting has not changed. Additional questions have been added to the forms in areas including PII training verification and use of the Social Security number, and a revised process is implemented for notifying the US Computer Emergency Response Team. These SECNAV forms may be used immediately, but must be used by 1 June 2016 for DON PII breach reporting.
