DoD IG Semiannual Report to the Congress | October 1, 2015 to March 31, 2016

The Semiannual Report to the Congress has been issued for the reporting period of October 1, 2015, to March 31, 2016. This report summarizes the work of the Office of Inspector General (OIG) from October 1, 2015, through March 31, 2016. As this Semiannual Report reflects, it has been a busy and productive reporting period. During the past 6 months the OIG issued a total of 73 reports.

Our Auditing component issued 57 reports that identified $1.6 billion in questioned costs and $82.4 million in funds put to better use. The OIG achieved $1.3 billion in financial savings based on management-completed corrective actions related to reports issued in previous reporting periods.

Our Defense Criminal Investigative Service (DCIS) opened 242 cases, closed 251 cases, and has 1,624 ongoing investigations. Cases addressed criminal matters such as procurement fraud, public corruption, product substitution, health care fraud, and illegal transfer of technology. DCIS investigations, including those conducted jointly with other law enforcement organizations, have resulted in $407.5 million in civil judgements and settlements; $83.2 million in criminal fines, penalties, and restitution ordered; $74.6 million in administrative recoveries; and $1.6 million in recovered Government property.

In our Administrative Investigations component, the DoD Hotline received 6,540 contacts, opened 2,973 cases, and closed 2,764 cases. Administrative Investigations also received a total of 339 senior official investigations and 797 whistleblower reprisal or restriction complaints in the first half of fiscal year 2016.

During this reporting period, our Special Plans and Operations component issued three reports, including a classified report on report on equipping and training the Sunni Popular Mobilization Forces in Iraq. Our Intelligence and Special Program Assessments component released three classified reports, including one on intelligence support countering weapons of mass destruction.

Our Policy and Oversight component issued 10 evaluation reports involving its oversight of audit, investigative, and technical issues in DoD involving oversight of audit, investigative, and technical issues in DoD.

The DoD OIG also continues its important work as the Lead Inspector General for two overseas contingency operations—Operation Inherent Resolve and Operation Freedom’s Sentinel. We continue to work closely with our OIG partners, particularly the U.S. Department of State OIG and the U.S. Agency for International Development OIG to provide coordinated oversight and reporting over these contingency operations.

DoD IG Semiannual Report to the Congress | April 1, 2015 to September 30, 2015

SAR Sept 2015 CoverThe Semiannual Report to the Congress has been issued for the reporting period of April 1, 2015, to September 30, 2015, issued in accordance with the Inspector General Act of 1978, as amended.   The report summarizes the scope of DoD IG’s oversight activities, as well as the contributions of the audit and investigative agencies of the military departments.

During the reporting period the DoD IG issued 80 reports that identified $384.4 million in questioned costs and $1.8 billion in funds put to better use. The DoD IG also achieved $180.8 million in financial savings based on management-completed corrective actions to reports issued during this and previous reporting periods. Auditing issued 57 reports with over 200 recommendations, identifying potential cost savings and funds that could be put to better use; ensure the safety of service members; address improvements in DoD operations, financial reporting, and accountability; ensure the Department complied with statutory mandates; and identify new efficiencies. DoD IG auditing reports addressed deficiencies with the Department’s acquisitions, management of contracts, financial management, audit readiness efforts, health and readiness of the total force, cyber operations, and equipping and training Afghan security forces. Investigations–Defense Criminal Investigative Service (DCIS) opened 268 cases, closed 282 cases, and has 1,625 ongoing investigations. Cases resolved in this reporting period primarily addressed criminal allegations of procurement fraud, public corruption, product substitution, illegal transfer of technology, health care fraud, and computer crime. Additionally, DCIS seized assets totaling $12 million, had final orders of forfeiture totaling $45 million, and monetary judgments in the amount of $54 million. Assets that have been seized include cash/currency, financial instruments, real property, and vehicles. Administrative Investigations (AI) received a total of 322 senior official and 643 whistleblower reprisal/restriction complaints in the during the reporting period; and closed a total of 425 senior official and 555 whistleblower reprisal restriction complaints. The DoD Hotline received 5,932 contacts, opened 2,250 cases, and closed 2,797 cases

DoD IG Semiannual Report to Congress October 1, 2014, to March 31, 2015

The Semiannual Report to the Congress has been issued for the reporting period of October 1, 2014, to March 31, 2015, issued in accordance with the Inspector General Act of 1978, as amended.   The report summarizes the scope of DoD IG’s oversight activities, as well as the contributions of the audit and investigative agencies of the military departments.

During this reporting period, the DoD IG issued 103 reports, identifying $101.1 million in questioned costs and $261.6 million in funds put to better use. We achieved $41.1 million in financial savings based on management-completed corrective actions to reports issued during this and previous reporting periods. Defense Criminal Investigative Service reported 57 arrests, 120 criminal charges, 101 criminal convictions, 121 suspensions and 120 debarments, as well as $921.9 million in investigative receivables and recoveries. An additional $123.6 million in assets were seized along with $12.7 million in monetary judgments.

The Administrative Investigations Component closed 437 senior official investigations and 617 whistleblower reprisal/restriction cases. The Defense Hotline received 5,932 contacts.

DoD IG Semiannual Report to the Congress | April 1, 2014 to September 30, 2014

The Semiannual Report to the Congress has been issued for the reporting period of April 1, 2014, to September 30, 2014. The report summarizes the scope of DoD IG’s oversight activities, as well as the contributions of the audit and investigative agencies of the military departments. During the reporting period, DoD IG issued 76 reports identifying $9 billion in potential monetary benefits.  Defense Criminal Investigative Service investigations were the basis for 70 arrests, 145 criminal charges, 136 criminal convictions, 68 suspensions and 137 debarments as well as $354.9 million in investigative receivables and recoveries. 

This report highlights a few of those significant projects completed during the reporting period, including the management of the Air Force’s MQ-9 acquisition program, the Defense Logistics Agency’s overpayment of spare parts for the Bell Helicopters, and an investigation of the owner and president of Precision Metal Spinning, Inc., for making false statements to the Government by the Defense Criminal Investigative Service, resulting in a $1.6 million settlement and suspension from contracting with the Government.

DoD IG Semiannual Report to the Congress | October 1, 2013 to March 31, 2014

The Semiannual Report to the Congress has been issued for the reporting period of October 1, 2013, through March 31, 2014. The report summarizes the scope of DoD IG’s oversight activities, as well as the contributions of the audit and investigative agencies of the military departments. During the reporting period, DoD IG issued 52 reports identifying $3.6 million in potential monetary benefits. Defense Criminal Investigative Service investigations were the basis for 48 arrests, 171 criminal charges, 147 criminal convictions, 82 suspensions, and 81 debarments, as well as $1.5 billion in monetary benefits to the Government.

This report highlights a few of those significant projects completed during the reporting period, including an assessment of the operations of Arlington National Cemetery and the Soldiers’ and Airmen’s Home National Cemetery, and an investigation by the Defense Criminal Investigative Service that resulted in a civil settlement of $3.5 million by Sikorsky Aircraft Corporation to resolve allegations of false claims.
