Each year, the DoD Patient Safety Program partners with the National Patient Safety Foundation to promote Patient Safety Awareness Week (March 13-19) and increase the awareness of patient safety resources available to leaders, providers and patients in the Military Health System.   

This year’s theme “United for Patient Safety” emphasizes the importance of patient engagement as a main factor to improve patient safety and reduce harm.    

From executive leadership to front-line staff, we are all encouraged to stand by our patients and partner with them to fulfill our goal of delivering the highest quality of care and and demonstrating that across the MHS, we are all “United for Patient Safety.”     

The seven days of the campaign also serve as time to recognize the range of work being done by our patient safety champions at military treatment facilities in the CONUS and OCONUS. For this reason, the DoD Patient Safety program invites you to engage and participate in the campaign by:

  • Accessing complimentary patient safety resources and tools, including webcasts, educational materials and publications – all available through the DoD PSP’s partnership with the NPSF and the Stand Up for Patient Safety Program. Learn more.   
  • Sharing your MTF’s PSAW success stories and highlights with the DoD PSP team by contacting DHA.patientsafety@mail.mil.   
  • Testing your “Patient Safety IQ” during the week of the campaign on the DoD PSP Facebook page.   
  • Accessing the Patient Safety Learning Center – a member-based Patient Safety Community of Interest that provides access to patient safety information, resources and best practices across the MHS. Learn more.   
  • Promoting the DoD Patient Safety Culture Survey throughout the duration of PSAW. The Culture Survey invites us all to take a stand for patient safety and PSAW is opportunity to let your opinions about patient safety at your MTF be heard. Learn more.

The awards program recognizes those who have shown initiative and commitment to the development of systems and processes that will lead the MHS toward a better, safer, nationally recognized health care system that all MHS leaders and staff strive for each and every day and one that all patients deserve. The connection between transparency and high reliability is one that touches many aspects of what we do as MHS patient safety professionals. We strive to be transparent in the way we share information with our patients – working with them as partners in their care and informing them of what we do openly and visibly. We are also transparent about our MTFs progress towards high reliability so that we can continue marching towards our goal of zero patient harm with efforts such as the Patient Safety Culture Survey. Another way that we, as MHS patient safety professionals, can display transparency on a daily basis is by using tools such as the I’M SAFE TeamSTEPPS Checklist.

HRO Corner: The Patient Safety Culture Survey Resource Guide – An Overview

Soldiers from the 399th Combat Support Hospital, 804th Medical Brigade, 3d Medical Command (Deployment Support) watch video footage of their performance during an exercise held April 2, 2016 at the Mayo Clinic Multidisciplinary Simulation Center in Rochester, Minnesota. The unit's every move was recorded at the facility, which allowed them to review and improve their performance throughout the course of the exercise. The DoD is on a journey to transform the Military Health System (MHS) into a high reliability organization (HRO) to ensure safe, reliable care for all of its patients and their families. This transformative shift is one that requires a laser-sharp focus by every one of us – leadership and frontline staff – to identify high-risk situations before they lead to an adverse event. To make this possible, prevention and performance improvement must become intrinsic elements of what we do on a daily basis. First, however, we must understand the progress each of our Military Treatment Facilities (MTFs) is making towards the high reliability goal.
