New Hires to the Federal Government

The following forms are required for all hires new to the Federal government. These forms are necessary to process your appointment into our personnel database.

Each of the following forms is a PDF file that can be filled in directly. Once completed, forms need to be mailed to the address indicated on your confirmation letter and must be received before your start date. We need original signatures on these forms.

OF-306: Declaration for Federal Employment

This form is used to determine your acceptability for Federal employment and your enrollment status in the Government's life insurance (FEGLI) program.

When completing the form, please ensure you include a detailed explanation for all questions where a "yes" answer is provided. Please note that a "yes" answer to one or more of the questions does not automatically exclude you from consideration for employment. For questions 9-11, please ensure all non-juvenile offenses, including alcohol-related incidents, are listed.

Upon completion of the form, please sign and date it as an Applicant (Item 17a). Please note you will not complete 17b and 18 (if applicable) until you are appointed to a position on your first day of employment.

If you have prior federal service, you will need to answer question 18 as well.

I-9: Employment Eligibility Verification

This form will be used to determine your eligibility to work in the United States. All NSWC Carderock division employees must be U.S. Citizens. All employees, hired after November 6, 1986, must complete section 1 of this form at the time of hire. The Immigration Reform and Control Act of 1986, Pub. L. 99-603 (8 USC 1324a) authorizes agencies to collect this information. Please complete Section 1. We will verify Part II on your first day of employment. You will need to bring your birth certificate and passport as proof of citizenship. In addition, please bring your social security card and driver's license on your first day.

SF-144: Statement of Prior Federal Service

Since some applications and resumes include only that employment history relevant to the position to which appointed, the SF-144 identifies any prior service for which records must be located. If your resume contains all of your PAID prior federal service and or military service, then you can check "Yes" in item 4, answer question 8 and sign and date in item 9. Completion of this form is required for all new hires even if you did not have any federal or military service.

SF-256: Self-Identification of Handicap

This form collects statistical data required by the Office of Personnel Management that is reported to the agency's personnel data system and to the Central Personnel Data File (CPDF). After the data is entered into the agency's system and verified/corrected, the form is destroyed.

SF-181: Race and National Origin Identification

29 CFR 1614.601 requires each agency to collect information on the race and national origin of its employees and to do so by employee self-identification.

FMS-2231: Fast Start Direct Deposit

This form is used for processing your net salary, allotments, and any other approved payments associated with Federal employment. Direct Deposit is required of all new hires. If you're unsure of your routing and account information, we recommend either verifying this information with your financial institution or attaching a voided blank check to this form.

W-4: Employee's Withholding Allowance Certificate

Complete Form W-4 so that your employer can withhold the correct federal income tax from your pay. Because your tax situation may change, you may want to refigure your withholding each year.

OPNAV 12910/1: Employee Reserve/Retired Military Category

This information will be used by the HRO to identify employees who could be recalled to military service in a mobilization effort and to plan for their loss to the workforce. If not applicable, please check "Y" for not applicable.

OPNAV 5239/14: System Authorization Access Request Navy (SAAR-N)

This form must be filled out and processed before you can have access to any Department of Defense (DoD) systems and/or information. You may fill in the PDF document electronically, but it must be printed and mailed/faxed.

Various State Tax Forms:

VA-4: Employee's Virginia Income Tax Withholding Exemption Certificate

Use this form to notify your employer whether you are subject to Virginia income tax withholding and how many exemptions you are allowed to claim. You must file this form with your employer when your employment begins. If you do not file this form, your employer must withhold Virginia income tax as if you had no exemptions.


MW-507: Employee's Maryland Withholding Exemption Certificate

This form is used to record the amount to be withheld from an employee's pay for Maryland state taxes.

D4: Employee's District of Columbia Withholding Allowance Worksheet

Every new employee who resides in DC and who is required to have DC taxes withheld, must fill out form D-4 and file it with his/her employer. This form is used to record the amount to be withheld from an employee's pay for District of Columbia taxes.
Transferred or Reassigned Hires from Another Activity

The following forms may be required for all hires transferring or reassigning from another government activity to the Naval Surface Warfare Center, Carderock Division. Your offer letter will instruct you which forms must be completed.
