Continued Health Care Benefit Program

Continued Health Care Benefit Program (CHCBP) costs depend on your sponsor's military status before he/she separated:

  • Active duty,
  • Guard/Reserve, or
  • Retired

Outpatient Deductible

You're required to pay the outpatient deductible before the CHCBP begins to cost share.

Sponsor's Former Status  Deductible Amount 
Active Duty  
  • Rank E4 and below: $50 per individual, but no more than $100 per family
  • Rank E5 and above: $150 per individual, but no more than $300 per family
Guard/Reserve 
  • Rank E4 and below: $50 per individual, but no more than $100 per family
  • Rank E5 and above: $150 per individual, but no more than $300 per family 
Retired 
  • $150 per individual
  • No more than $300 per family 

Cost Shares

After you meet your deductible, you'll pay a cost share based on the type of care and type of provider you see (network vs. non-network). Your CHCBP cost share depends on your beneficiary category before your sponsor separated:
