The SHWC provides appointments for students traveling abroad for a UNT trip or personal travel. To schedule an appointment call (940) 565-2333 or schedule your appointment online. When making your appointment make sure you tell them where you are going on your trip. You may be required to make a 2nd appointment if necessary.
What to bring to your appointment:
- SHWC Pre-Travel form
- Immunization records
- Travel forms
- Student ID card
Immunizations must be given at least 14 days prior to traveling to be effective.
List of Immunizations offered at the SHWC:
- Hepatitis A
- Hepatitis B
- MMR( Mumps, Measles and Rubella)
- Tetanus
- Adacel (Tdap)
- Meningitis
We also offer oral Typhoid and Malaria pills in the UNT Pharmacy with a prescription from a medical provider.
Please note, there is a $10 charge if the student has to have forms filled out.
