Adoption

The Texas Vital Statistics Adoption Team works with attorneys and district clerks to process adoptions in Texas. If you are an attorney or district clerk and need a sample adoption-related document, click here for our list of forms.

Click here for frequently asked questions related to adoption. Please note that because each situation is different, every application must be reviewed by a specialist to determine whether it is acceptable. If the application is determined to not be acceptable, you will be contacted with additional instructions, which will increase the processing time.

Adult Adoptee Information
