10/7/2014 MI: Worker trimming tree killed in fall from lift.

10/16/2014 NC: Worker died from heat stroke.

10/23/2014 IL: Temporary worker killed in fall from third floor loading platform.

10/24/2014 OH: Two workers killed in fall from scaffold.
