Passport applications are accepted at the County Clerk's
office in Denton, our satellite office located in the
Sandy Jacobs Government Center - Carrollton
and the satellite office located in the
Steven Everett Copeland Government Center - Crossroads.
U.S. Passport Application for
Children under Age 16
Requirements:
- All applicants must
apply in person
- Certified birth certificate with State, City or County Seal
or previous U.S. Passport, or Certificate of Naturalization (original only)
or Certificate of Citizenship or Consular Report of Birth Abroad, Hospital birth
certificates ARE NOT ACCEPTABLE.
- Two identical recently made 2" x 2" photos with
image size between 1 and 1 3/8 inches from bottom of chin to top of hair with plain white
background. Photos must be clear, front view, full face, taken in normal street attire.
(No hats or dark glasses). PHOTOS ARE AVAILABLE AT THE COUNTY CLERKS OFFICE FOR
$10.00 CASH ONLY (includes both pictures).
- Valid drivers license or military ID.
- Applicants 17 years of age and under must appear in person
with a parent or legal guardian. Application for minors under the age of 16 must be done
in person by both natural parents who are named on the birth certificate.
- Certified proof of name change (required only when
applicable).
- Your Social Security number or social security number of
child applying for passport.
Fee Schedule:
- $75.00 CHECK OR MONEY
ORDER ONLY, payable to US Department of State, for
16 years and older. $60.00 CHECK OR MONEY ORDER ONLY,
for 15 years and under.
- $25.00 CASH ONLY for County Clerk execution fee.
- $10.00 CASH ONLY for photos (optional)
Total fee for 16 and over (adult) passport for
all services: $110.00
Total fee for 15 and under (minor) passport
for all services: $95.00
Expediting is available for $60 additional.
This should be added into your money order or check.
* Proof of departure is only required when using a private expediting service.
Time Needed to Process Applications:
- In the Denton location, complete passport applications cannot be
processed after 4:45p.m. that business day. In the Carrollton and Crossroads locations, passport applications
cannot be processed after 4:10pm that business day.
- Once we receive the application, with Regular Passport
Service expect to receive your passport in approximately 5 - 6 weeks. This time varies
greatly dependent upon whether it is a busy travel season.
- If you choose to pay for Expedited Passport Service, expect
to receive your passport within 2 - 3 weeks.
Passport Applications:
It is possible to download passport application forms in
Portable Document Format (PDF). Adobe PDF files require Acrobat Reader software from Adobe
Systems Inc. The forms you print must be clear and sharp. Applications that are
blurred or illegible will not be accepted. They must be printed in black print on white
paper. The paper must be 8 1/2 inches by 11 inches, with no holes or perforations, at
least medium (20 lb.) weight, and with a matte surface. Thermal paper,
dye-sublimation paper, special inkjet paper, and other shiny-papers are not

DSP-11 Form -(First
time applicants & minors) Download
This form must be presented and signed in person at an
authorized passport acceptance facility. If you have never had a U.S. passport, you are
under age 18, or if your most recent passport was lost or stolen you must use this form.
(If you are applying because your passport was lost or stolen, you will also need to
submit Form DSP-64.)
DSP-82 Form (Renewal of
Passport) Download
(For Renewals Only) This form should be used for renewing a
previously issued U.S. passport by mail. There are four criteria for
using this form. Please use the following checklist to see if you are eligible to apply
with a DSP-82:
1. I can submit my most recent passport.
2. I was at least 16 years old when my most recent passport was issued.
3. I was issued my most recent passport less than 15 years ago.
4. I use the same name as on my most recent passport, OR, I have had my name changed by
marriage or court order and can submit proper documentation to reflect my name changes.
