It is the responsibility of the student to complete the Consent to Treat Form after you register for Eagle Camp. You MUST mail or fax this form to the Eagle Camp Staff prior to your arrival for Eagle Camp. While we hope we never have to use it, students are not allowed to attend Eagle Camp without this form on file.
Camp Financial Assistance Form

Students can apply for financial assistance if they cannot attend Eagle Camp due to the cost. To apply, complete the financial assistance form prior to registering for Eagle Camp. For students that are granted financial assistance, we can waive the registration fee. These funds are limited, so please complete the form if the fee is truly holding you back from attending. Please download the Financial Assistance form (pdf) and either mail or fax it to us before the registration deadline.
