3320 Temporary Work Permits

 

 

1.0          Purpose

 

Certain hazardous, recurring activities performed at Jefferson Lab are restricted and only undertaken when authorized by a temporary work permit.  These work permits ensures that all hazards have been considered and associated risks have been acknowledged and mitigated. 

 

The following activities have been identified as requiring a temporary work permit prior to execution:

 

·       Permit-Required Confined Space Entry

·       Energized Electrical Work

·       Fire Hazardous Work

·       Radiological Work

·       Dig/Blind Penetration

 

2.0          Scope

 

Because of the severe consequences which could occur during the above noted activities, they have been provided for in separate documents as noted above.  These procedures, as outlined, provide for a thorough hazard evaluation.

Responsibilities are outlined within the associated documents.

 

4.0          Referenced Documents

 

4.1            Confined Space Entry Permit (See ES&H Manual Chapter 6160 Confined Space Entry)

According to OSHA a permit-required confined space (PRCS) has the following characteristics:

·       Sufficient space for a person to enter and perform work;

·       Limited/restricted means for entering/exiting;

·       Not designed for continuous occupancy.

 

And one or more of the following criteria:

·       Contains, or has the potential to contain, a hazardous atmosphere;

·       Contains a material with the potential for engulfing a person or hindering quick escape;

·       Internal configuration can trap a person or lead to asphyxiation;

·       Entails a safety or health risk that has been determined to be RC >2 and requires a formal Task Hazard Analysis and associated work control document.

 

At Jefferson Lab these areas require a Confined-Space Entry Permit prior to entry.  See ES&H Manual Chapter 6160 Confined Space Entry for guidance.

 

4.2            Energized Electrical Work (See ES&H Manual Chapter 6220 AC Electrical Equipment Safe Work Policy)

Service work performed on AC electrical power distribution system at Jefferson Lab with equipment in an energized state requires an approved Energized Electrical Work Permit as described in ES&H Manual Chapter 6220 AC Electrical Equipment Safe Work Policy.  The permit establishes the justification for working on “live” equipment, the required training qualifications of the workers, and any unusual aspects of the work.  See ES&H Manual Chapter 6220 Appendix T1 Energized Electrical Work Permit (EEWP).  Note: Mode 3 work on Class II or above electrical power distribution equipment requires the approval of the Laboratory Director.

 

4.3            Hot Work Permit (See ES&H Manual Chapter 6900 Appendix T1 Fire Protection: Hot Work Permit)

Work that presents an inherent risk of creating a fire is restricted at Jefferson Lab.  Open-flame and spark-producing activities such as welding or soldering, are authorized by a Hot Work Permit.  See ES&H Manual Chapter 6900 Appendix T1 Fire Protection: Hot Work Permit.

 

4.4            Radiological Work (See Jefferson Lab Radiological Control Supplement)

Activities which present the possibility of significant exposure to ionizing radiation are restricted at Jefferson Lab.  Such activities are allowed only after careful analysis and planning to minimizing personnel exposure.  Exposure must be As Low As Reasonably Achievable (ALARA).

 

Jefferson Lab’s Radiation Control Group initiates and authorizes Radiological Work Permits.  It informs workers of entry requirements and radiological conditions in the work area, and provides mechanisms to reduce worker exposure.

 

A Radiological Work Permit is required for any of the following situations:

·       The anticipated radiation dose rate to the trunk of a worker's body – usually measured 30 cm (1 ft) from the work piece – is greater than 25 mR/hr

·       The total estimated dose to a worker exceeds 25 mrem

·       The workpiece measures 250 mR/hr or greater on contact

·       Entry into a posted contamination, High Radiation Area, or Airborne Radioactivity Area is necessary

4.5            Dig/Blind Penetration Permit (See Facilities Management’s Excavations and Blind Penetrations into Walls & Floors)

Excavation, digging, boring into soil; drilling or cutting blind penetrations into floors and wall; and demolition at Jefferson Lab is performed only after utilities have been located and marked by a competent person.

 

If you have work that requires digging or drilling into soil or blind penetration into walls or floors with tools or fasteners, contact Facilities Management (x7400) to complete a Dig/Blind Penetration Permit.
