| Funding for the Adolescent Parenting Program (APP) is currently provided to FamCare, Inc., Bridgeton, NJ to provide case management services with home visitations and coordinated referral services to a caseload of 85 pregnant and/or parenting teens and their infants who live in Cumberland County. The program supports the following four goals:
- Reduce rapid repeat births among adolescent parents in the 24-month post partum period.
- Improve parenting knowledge and behavioral life skills.
- Increase the number of parenting teens who attend or complete high school/GED or home study programs, and/or who are able to obtain sustainable employment.
- Increase the number of parenting teens and their children who have a primary care provider to ensure positive health status.
The case management staff consists of professional social workers and a registered nurse.
