Signs of a Stroke

Learn the warning signs and how to recognize a stroke.

For several years, a formula for recognizing a stroke has been circulating on the Internet. A number of individuals have asked us whether the advice is valid or not. The advice includes asking someone who appears to be having a stroke to perform several actions: smile; raise both arms; speak a simple sentence. If the person is unable to perform any of these tasks in a normal fashion, it recommends that the evaluator “dial 9-1-1 immediately and describe the symptoms [actually signs] to the dispatcher.”

The recommendation is valid. Both the American Heart Association and the American Stroke Association have stated in their websites and publications that this is a simple and effective screening method for the public to learn and use. Note, however, that a person’s ability to perform these tasks does not absolutely rule out the possibility of stroke; if there are other signs or symptoms such as sudden headache, dizziness, visual disturbances, loss of balance, confusion, or trouble understanding commands or questions, the evaluator should carry out the same actions: dial 9-1-1 and speak with the dispatcher.

Two other pieces of invalid advice have commonly coexisted with the above. One is the “stick out your tongue” test. Although deviation of the tongue while trying to stick it straight out could be a sign of a stroke, there is such variation in the ability to do so that the test has little accuracy. The other, unrelated and invalid suggestion is the so-called “cough self-resuscitation” technique; this has no value whatsoever and further description is excluded here lest it be taken as a recommendation.
