Attached is the new Motor Vehical Inspection form dated Oct 2011.

 

A new DD Form 626 has been issued. It changed the text of line 22 in Section III from "SHIPPING PAPERS/DD FORM 836 FOR GOVERNMENT VEHICLE SHIPMENTS" to "SHIPPING PAPERS/DD FORM 2890 FOR GOVERNMENT VEHICLE SHIPMENTS" and changes the requirements for military vehicles to have one vs two fire extinguishers.

    10.23.2. Provide each explosives-laden vehicle used for transport with at least two portable 2A:10BC rated extinguishers. One extinguisher will be mounted on the exterior driver’s side of the vehicle and one will be mounted inside the cab, readily accessible to the driver. If explosives-laden vehicles are parked at an explosives location, additional fire extinguishers beyond those required in paragraph 10.23 are not required. If the vehicle leaves the explosives location, additional extinguishers are required.

    Please note that the regulation has changed

    (49 CFR 393.95)

    Emergency equipment on all power units.
    Each truck, truck tractor, and bus (except those towed in driveaway-towaway operations) must be equipped as follows:
    (a) Fire extinguishers—(1) Minimum ratings. (i) A power unit that is used to transport hazardous materials in a quantity that requires placarding (See §177.823 of this title) must be equipped with a fire extinguisher having an Underwriters' Laboratories rating of 10 B:C or more

    As regards the send part of the above post, the change to the number of fire extinguishers required, I belive that a form cannot override a Regulation, and per DA PAM 385-64, 2 fire extinguishers are still required for military vehicles transporting ammunition ( 20–8. Outbound motor shipment of ammunition and explosives

    a. All motor vehicles which will be carrying DOT Class 1.1, 1.2 or 1.3 explosives must be inspected using a DD
    Form 626. Deficient equipment will not be used.

    b. When a commercial vehicle fails an inspection using a DD Form 626, a copy of the inspection will be provided
    to the transportation officer and the procedures in DTR 4500.9–R will be followed.

    c. Loading methods prescribed by Army Materiel Command (AMC) 19–48 series drawings will be followed for the
    loading and bracing of motor vehicle shipments of military ammunition and explosives. The packages will be placed in position without excessive or violent force.

    d. All Government trucks transporting any DOT class of explosives (both on post and off post) will be equipped with two portable fire extinguishers rated class 10BC or greater. It is recommended that these two fire extinguishers be rated 2A:10B:C to enable users to fight a class “A” fire. One must be CO2 or dry chemical, if chemical munitions are being transported. Commercial trucks transporting explosives for the Army are required to have only one 10BC or greater rated fire extinguisher. Crews loading and unloading vehicles carrying or about to carry ammunition or
    Explosives must have two 10BC or greater fire extinguishers available as required by chapter 6.
