1.0 Introduction

This Subject Area is applicable to all Office of Science (SC) Site Offices and SC Headquarters (HQ). These procedures describe Environment, Safety and Health (ES&H) reports which must be submitted by the SC Site Offices and, as applicable, by HQ Managers to SC HQ. These reports are in addition to the reporting requirements in U.S. Department of Energy (DOE) Directives (see Section 5.0, Requirements).

1. Notification of Reportable Occurrences
  • Notify Office of the Director, Office of the Deputy Director for Operations, and Office of Safety, Security, and Infrastructure of reportable occurrences.
2. Site Office Quarterly Safety Reports
  • Submit laboratory injury data to Computerized Accident/Incident Reporting System (CAIRS).
  • Submit Site Office Quarterly Safety Report.
