1.0 Applicability
This Subject Area procedure is applicable to all Office of Science (SC) Site Offices and SC Headquarters. This procedure describes Site Office quarterly safety reports which must be submitted by the SC Site Offices to SC Headquarters.
2.0 Required Procedure
Step 1 | The Site Office ensures that laboratory injury data is submitted to the Computerized Accident/Incident Reporting System (CAIRS) by the 15th of the month following the end of the quarter. |
Step 2 | The Site Office Manager reviews and approves the Site Office Quarterly Safety Report and provides it to the Director, Environment, Safety and Health (ES&H) Division by the 21st of the month following the end of the quarter. |
Step 3 | ES&H Director reviews the Site Office Quarterly Safety Report and provides feedback to the Site Office Manager. The Site Office will revise and resubmit the report based on the feedback from the ES&H Director. ES&H Director supplies copies of the final Quarterly Safety Reports to the Site Office Managers. |
Step 4 | SC senior management is briefed on the Quarterly Safety Reports for all SC sites. The briefing may be done by the Site Offices via teleconference, videoconference, and/or by the ES&H Director. |
Step 5 | After SC senior management is briefed, ES&H Director uploads a copy of the final injury/illness rate data to the SC Safety Program Web Page. |
