The Office of Quality and Safety is now called the Office of Quality Safety and Value (QSV).

QSV is a consolidation of Quality Management, Safety, Systems Redesign, Improvement, and Compliance resources into a single office to help enhance the quality, safety, reliability and value of clinical and business systems by enabling innovative, enterprise-wide approaches to compliance, risk awareness and continuous improvement.
