FMA Vessel/Plant Operator Comment Form

Thank you for taking the time to fill out this Vessel/Plant Operator Comment Form. It is identical in content to the paper copy that we are using. This form allows you to provide us with feedback regarding your recent Observer and your experience with the Observer Program as a whole. Your feedback is important to us. We are committed to responding to each Comment Form we receive.

Things to note:

1. In order to fill out this form, you must have either Adobe Acrobat Reader software installed or a version of Adobe Acrobat Professional.
2. Once you open the Vessel/Plant Operator Comment Form, you can either scroll down and use the mouse to move through the form or use the Tab key on your keyboard to move forward through the editable portions of the form (pages 2 and 3). Need extra help?
3. Once you have completed this form, please click the Save Form button, save the file (with a different file name), and email it as an attachment to: patti.nelson@noaa.gov
4. If you prefer to mail a hard copy to us instead, click the Print Form button to print a copy of your completed form, and mail it to:
