Effective June 1, 2012, all DUA request submissions must follow the e-mail Subject line instructions located on the DUA-Forms page (use the link in the left hand menu).
For all new Data Use Agreement (DUA) requests, use the links in the left margin for specific instructions on making requests for CMS data.
Free assistance is available for:
- Federal Agencies (excluding CMS contractors/grantees)
- Federal Grantees
- Medicare Current Beneficiary Survey (MCBS) data requestors
- Researchers (RIFs) (excluding Providers, LDS and DSH requesters)
