The information shown in box 10 of your W-2 is the total amount of dependent care benefits that your employer paid to you or incurred on your behalf. Any amount over $5,000 ($2,500 in the case of a separate return filed by a married individual) is also included in box 1. You may be able to exclude these benefits from your
income.

Form 2441 (PDF),
Child and Dependent Care Expenses, to claim the exclusion. You cannot use
Form 1040EZ (PDF).
