Onboarding Documents and Forms

Before you begin your new career as a Civilian Marine, we will require you to fill out and sign a number of forms that have been mandated by law. You will be asked to present these forms on your first day.

We recommend that you submit this paperwork in advance, however, you may bring some or all of the paperwork with you to your Entrance on Duty (EOD).

Important Note Regarding Email

There is a small but real possibility that documents emailed over unsecured internet connections may be intercepted. The Federal Government cannot ask or require that documentation containing personally identifiable information (PII) such as Social Security Numbers be submitted via email. You may, however at your own risk scan and email any or all of the required documentation into the Recruiting and Classification Section office, if you so choose.

Document Submission Instructions

Please download, print, and complete the forms indicated below and (if possible) fax them to the following number at Marine Corps Base Quantico:

Fax: (703) 784-2048
Attn: Recruitment/Classification Section

Cautions regarding mailing: Please note that, because of delays in the handling of mail between you and the Recruitment/Classification Section office, it may take up to four weeks to receive your submissions. For this reason, we discourage mailing your forms, unless you are certain that you have at least four weeks before your Entrance on Duty date. If you feel you must send your submissions through the postal service, we recommend using Priority Mail, Certified, Return Receipt Requested. We cannot be responsible for documentation that may be lost in the mail. Our postal address is:
