If you would like to submit a request or appeal to OIP through this online request form you will be able to set up a personal account. You will be asked to provide your name and contact information and to create a unique username and password for this site. Your unique user account will allow you to log into this site and track any request or appeal that you have made to OIP. By submitting a request or appeal through this online request form, OIP will then communicate with you through your online account. OIP's determination, as well as any responsive documents, will be provided to you electronically, through your secure, online account.
When OIP first receives your request or appeal, your account will be updated with a holding number until OIP's staff assigns your request or appeal an individualized tracking number. You will also receive an electronic copy of OIP's acknowledgement letter. As OIP processes your request or appeal, OIP's staff will update the status of your request or appeal in your account. You can access your account at any time to check the status of your request or appeal. When an interim or final determination is made in response to your request or appeal, OIP will post an electronic copy of its response letter and any responsive documents in your account.
If you prefer not to use this capability you may still submit your request or appeal to OIP by mail, fax, or e-mail.
