Title:	Care Transitions: “Time to Come Home” (March 2011)
Abstract:	This report identifies potential problem areas within patient care coordination during the transition from inpatient care back to the rural community. It focuses on communications from larger referral hospitals back to rural residents and their primary care providers, describes characteristics of patient interactions with physicians and the areas where care transitions occur, discusses the various care transition situations and the challenges the rural environment presents, and describes major components of a tool to measure the quality of the discharge process and transition back to the community. The report includes a table, references, and an appendix listing transfer communications expert panel members.
