Direct Deposit Sign-Up Form
*So that your earnings may be deposited directly in your bank account.

A separate form must be completed for each type of payment to be sent by Direct
Deposit.

in this form. Also, see website for frequent program/policy updates.
Please note that the paper application is only to be used if you do not or will
NOT have a CAC or access to a CAC enabled system. Please note that
this does not apply to those who are slated to receive a CAC or will be gaining
computer access soon. If you are a new hire and will be receiving a CAC in the
near future, you are required to use the web-based application, so please wait
to apply for your transit benefits until that point so that you may utilize the
web-based application so that we can comply with the OMB mandates and DOD policy
regarding this program.
