Once you have your list, select any of the forms below and follow the instructions regarding each:


SF-86:  Questionnaire for Sensitive Positions

*This document is sensitive and will be mailed to you from our security department.  When you receive it, follow the instructions accordingly.

SAAR-N:  System Authorization Access Request Navy

*Must be processed before you will have access to any DoD systems and/or information.  You may fill in the PDF document electronically, but it must be printed and mailed/faxed.

Form I-9:  Employment Eligibility Verification

*To attest that you are a citizen/national or lawful permanent resident of the United States and thus eligible for employment.  Fill, print, and bring with you on your first day.

OF-306:  Declaration of Federal Employment

*Used to determine your acceptability for Federal and Federal contract employment and enrollment status in the Government’s Life Insurance program.  Fill, print, and bring with you on your first day.

SF-144:  Statement of Prior Federal Service

*Providing this information is voluntary, but failure to do so may result in your not receiving credit for prior Federal service.  Fill, print, and bring with you on your first day.

*For residents of the District of Columbia.  Print, fill, and bring with you on your first day.

*For residents of the state of Maryland.  Print, fill, and bring with you on your first day.

*For residents of the state of Virginia.  Print, fill, and bring with you on your first day.

W-4:  Federal Withholding Tax Form

*To be completed so that NAVSEA can withhold the correct federal income tax from your pay.  Should be completed each year and when your personal or financial situation changes.  This PDF form can be filled electronically and saved.  Print a copy and bring on your first day.

SF-1199:  Direct Deposit Sign-Up Form

*So that your earnings may be deposited directly in your bank account.  Fill, print, and bring with you on your first day.  A separate form must be completed for each type of payment to be sent by Direct Deposit.

*Please see block two, “General Instructions”, for important information on how to turn in this form.  Also, see website for frequent program/policy updates.   Please note that the paper application is only to be used if you do not or will NOT have a CAC or access to a CAC enabled system.   Please note that this does not apply to those who are slated to receive a CAC or will be gaining computer access soon. If you are a new hire and will be receiving a CAC in the near future, you are required to use the web-based application, so please wait to apply for your transit benefits until that point so that you may utilize the web-based application so that we can comply with the OMB mandates and DOD policy regarding this program.

*Form to acquire on-site employee parking at WNY.  Print, fill, and bring with you on your first day.  Please note the directions on page two; overflow parking information on page three; and towing information on page four.

SF-256   Self-Identification of Handicap

*Self-identification is voluntary, but your cooperation in providing accurate information is critical for effective data collection and analysis.  Statistical purposes only.  Fill, print, and bring with you on your first day.

SF-181:  Ethnicity and Race Identification

*Information used strictly for research/survey and Equal Opportunity Employment planning purposes.  Will not have an effect, adverse or otherwise, on employment.   Voluntary.  Fill, print, and bring with you on your first day.
