Privacy Act Statement

Authority:   5 U.S.C. 301; Executive Order 13254, “Establishing the USA Freedom Corps”

(January 29, 2002).

Purpose:  FEMA is collecting this information to provide state and local contact information for Citizen Corps Councils and Community Emergency Response Team (CERT) Programs to members of the public who are interested in becoming more prepared and/or for getting involved in community activities.  This information also showcases the capabilities of each local Council and CERT Program as well as recognizes them as an approved entity within the state. 

Routine Uses: This information may be disclosed as generally permitted under 5 U.S.C. § 552a(b) of the Privacy Act of 1974, as amended, including disclosing this information as necessary and authorized by the routine uses published in DHS/ALL– 006 Citizen Corps Database (December 19, 2008, 73 FR 77785) (//www.gpo.gov/fdsys/pkg/FR-2008-12-19/html/E8-29809.htm) and upon written request, by agreement, or as required by law.

Disclosure: Furnishing this information is voluntary; however if you choose not to provide the information, FEMA may be unable to complete your Citizen Corps or CERT registration or exchange information about, or facilitate your involvement in Citizen Corps or CERT  initiatives.

PAPERWORK BURDEN DISCLOSURE NOTICE

Citizen Corps Council Registration & Public reporting burden for this data collection is estimated to average 1 hour per response. The burden estimate includes the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed and completing and submitting this form. You are not required to respond to this collection of information unless a valid OMB control number is displayed on this form.
