Evacuation Information Packet

Please download the following Evacuation Information Packet forms. Downloadable versions of each form are linked to the individual form names that are identified by blue font listed below. You may not be able to fill out every form completely, but please at least print a blank or partially filled copy of every form. Please note that, some information will not be available unless an evacuation was to occur.







PART I: INTRODUCTION AND EVACUEE PREPAREDNESS

PART II: IDENTIFICATION (FAMILY AND PETS)

  • ID card/CAC

  • Social Security Cards
  • Birth Certificates and Adoption Papers
  • Marriage License and Divorce Decree
  • Valid U.S. Driver’s License
  • Powers of Attorney
  • Family Care Plan (The below forms are to be used if you do not already have a completed family care plan)
  • DD Form 2208: Pet Vaccination Record (three copies in a waterproof envelope for the airline approved pet carrier)
  • DD Form 2209 Veterinary Health Certificate (three copies in a waterproof envelope for the airline approved pet carrier)

PART III: EVACUATION FORMS AND ORDERS

  • PCS Orders and Area Clearance (or Command Sponsorship paperwork)
  • DD Form 1610 Request and Authorization for TDY/TAD Travel (for Military Personnel) May be required during the evacuation process. Lines of accounting and approval signatures will be provided at evacuation processing centers.
  • Evacuation Orders May be provided by your unit administrative staff, official message traffic, locally generate order, or DD Form 1610
  • DD Form 2585 Repatriation Processing Center Processing Form Required during in-processing for personnel accountability while in a safe haven status. This information is entered into a family support system and ensures families can be kept apprised of latest news regarding changes in status. It is critical that this document is complete.
  • Letter of Authorization to Evacuate to a Country Other than the United States Family members who must go to a country other than the U.S. will need a letter prepared ahead of time. This option is designed for those who don’t have family in the U.S. and foreign spouses. Once the family arrives at the repatriation site, their ticket(s) should be available.

PART IV: FINANCE AND HOUSEHOLD GOODS

  • DD Form 2461 Authorization for Emergency Advances and Allotment Pay for DOD Civilians Used by non-command sponsored DOD civilians to authorize payment of emergency funds to their family members in case of evacuation from Okinawa.
  • DD Form 1337 Authorization/Designation for Emergency Pay and Allowance Used for families of military personnel. Without the appropriate form, required signatures, and in the case of civilians, appropriate sponsor’s fund site, families may not be able to receive an advance pay from the sponsor’s pay entitlements.
  • DD Form 1701 Inventory of Household Goods This form will be used as an inventory of household goods located in the evacuee’s quarters in Okinawa. This form may be used later upon return to the U.S. if it becomes necessary to file a claim against the U.S. government for personal property lost and/or destroyed as a result of the evacuation from Okinawa. You can use the detailed list of household goods from your most recent PCS move in place of this form.

PART V: AUTOMOBILES AND RESIDENCE

  • Vehicle Key Tag Provides authorities who remain on Okinawa a means to safely relocated vehicles
  • Residence Key Tag Provides authorities who remain on Okinawa a means to access your residence for security reasons or to ship your household goods if possible
  • Vehicle Insurance (American)
  • Vehicle Insurance (Japanese)
  • USFJ Form 207 Military Vehicle Registration (2 copies)
  • Certificate of Title of Motor Vehicle (2 copies)

PART VI: OTHER IMPORTANT DOCUMENTS AND CONSIDERATIONS

  • DD Form 93 Record of Emergency Data (RED) Required to help establish next of kin. Ensure that this is up to date and that a copy is located within the Evacuation Information Packet
  • Copies of Medical and Dental Information
  • Immunization Records
  • Insurance (health, life, etc)
  • Financial Records (checkbook/bank books, credit cards, tax records, current bills)
  • School Records (transcripts, test scores, etc)
  • Employment Records (resume, pay vouchers, SF50, Notification of Personal Action, latest performance evaluation or appraisal, etc)
  • Prescriptions for Important Medications
  • Last Will and Testament
  • Important Contacts including essential Unit phone numbers Tailor contact list to meet your needs
  • DD Form 2258 Temporary Mail Disposition Instructions Redirects mail from your Okinawa address to your new address
  • Funds for the transportation of pets If the government is able to evacuate your pets, you will be responsible for the transportation costs from the repatriation site to your home of record
  • Approximately $100 cash per person (U.S. dollars and yen)
