Item No.: 08
Site: Potable Water-Cross-Connection Control Program
Violation: The cross-connection control program listed the non-testable backflow prevention devices with an inspection date column where the staff wrote N/A for not applicable. All air gaps, double check valves with intermediate vents, and atmospheric vacuum breakers were not included in a periodic inspection schedule. In addition, the listing described the Watts N9C as pressure vacuum breaker for non-continuous pressure applications; the Watts N9C is a double check valve with intermediate vents for continuous pressure applications.
Recommendation: Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location. Include air gaps on the listing. Ensure a schedule for inspection frequency is set. Maintain a log documenting the inspection and maintenance in written or electronic form, and ensure it is available for review during inspections.

Site: Recreational Water Facilities-Hair and Lint Strainers
Violation: Staff stated that the hair and lint strainers and hair and lint strainer housings on the recreational water facilities were cleaned and disinfected but there was no documentation regarding the frequency of cleaning, disinfectant concentration, or contact time.
Recommendation: Clean, rinse, and disinfect the hair and lint strainer and hair and lint strainer housing on all RWFs weekly. Ensure disinfection is accomplished with an appropriate halogen-based disinfectant. At a minimum, use a 50-ppm solution for 1 minute, or equivalent CT value. Maintain records on all inspection and cleaning procedures.

Site: Recreational Water Facilities-Filter Housings
Violation: Staff stated that the filter housings on the recreational water facilities were cleaned and disinfected but there was no documentation regarding the frequency of cleaning, disinfectant concentration, or contact time.
Recommendation: Clean, rinse, and disinfect the filter housing before the new filter media is placed in it. Ensure disinfection is accomplished with an appropriate halogen-based disinfectant. At a minimum, use a 50-ppm solution for 1 minute, or equivalent CT value. Maintain records on all inspection and cleaning procedures.

Violation: The safety sign posted at the spa pool did not contain all elements of the 2011 VSP Operations Manual sign requirements. The signs did not include bather load or caution against use by individuals who are immunocompromised, on medication, or who have underlying medical conditions such as cardiovascular disease, diabetes, or high or low blood pressure. In addition, the safety signs for the whirlpool spas stated that children under the age of 14 are required to have adult supervision instead of cautioning against the use by children.
Recommendation: Children are defined as 16 years and below (2011 VSP Operations Manual clairification)

Site: Recreational Water Facilities-Safety Signs

The signs did not include bather load or caution against use by individuals who are immunocompromised, on medication, or who have underlying medical conditions such as cardiovascular disease, diabetes, or high or low blood pressure. In addition, the safety signs for the whirlpool spas stated that children under the age of 14 are required to have adult supervision instead of cautioning against the use by children.
Recommendation: In addition to the safety sign requirements in section 6.7.1.1.1, install a sign at each whirlpool spa and spa pool entrance listing precautions and risks associated with the use of these facilities. Include, at a minimum, cautions against use by the following: (1) individuals who are immunocompromised; (2) individuals on medication or who have underlying medical conditions such as cardiovascular disease, diabetes, or high or low blood pressure; and (3) pregnant women, elderly persons, and children. Additionally, caution against exceeding 15 minutes of exposure.

Site: Recreational Water Facilities-Safety Signs

Recommendation: Provide safety signs for all RWFs, except for baby-only water facilities. Ensure the signs, at a minimum, include the following words: (1) do not use these facilities if you are experiencing diarrhea, vomiting, or fever; (2) no children in diapers or who are not toilet trained; (3) shower before entering the facility; and (4) bather load #. For children?s RWF signs, include the exact wording ?TAKE CHILDREN ON FREQUENT BATHROOM BREAKS? or ?TAKE CHILDREN ON FREQUENT TOILET BREAKS.?

Site: Recreational Water Facilities-Automated Monitoring
Violation: The whirlpools did not have chart recorders or electronic data loggers installed on sample lines directly from the recreational water facilities or on the return lines from the recreational water facilities before the compensation tanks.
Recommendation: Ensure the sample line for the analyzer probe (monitoring) is either directly from the RWF or on the return line from each RWF and before the compensation tank.

Site: Recreational Water Facilities-Pool Shepherd's Hooks
Violation: The shepherd's hooks at the two swimming pools were telescoping.
Recommendation: Ensure it is a light, strong, nontelescoping material with rounded, nonsharp ends.

Site: Food Service General-Consumer Advisories
Violation: The menu's for the Manhattan Dining Room, Tamarind Restaurant, and Pinnacle Grill had consumer advisories indicating that consuming raw or undercooked animal derived foods may increase the risk of foodborne illness, especially to consumers with certain medical conditions. However, the animal derived products that could be ordered raw or undercooked were not identified with an asterisk.
Recommendation: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish that is raw, undercooked, or not otherwise processed to eliminate pathogens is offered in a ready-to-eat form or as a raw ingredient in another ready-to-eat food, ensure the consumer is informed by way of disclosure using menu advisories. Ensure the advisory is posted on a menu using an asterisk at the animal-derived foods requiring disclosure and a footnote with a statement indicating that consuming raw or undercooked meats, seafood, shellfish, eggs, milk, or poultry may increase your risk for foodborne illness, especially if you have certain medical conditions.

Site: Buffet-Deck 9 - Aft Beverage Station
Violation: Two trays of uncovered cups of ice water were stored unprotected on the counter of the beverage station. The inspector was informed that these glasses of water are only served by the beverage attendant and were not for passenger self-service. During this time, however, a passenger was observed serving themself.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected.

Site: Provisions-Deck B - Vegetable Preparation Room
Violation: The gasket to the potato exit chute on the deck mounted potato peeler was not properly attached.
Recommendation: Food contact equipment must be maintained in good repair and proper adjustment: (1) Equipment must be maintained in a state of repair and condition that meets the materials, design, construction, and operating specifications of these guidelines.

Site: Provisions-Deck B - Fish Preparation Room
Violation: There was excessive and peeling sealant at the backsplash/bulkhead juncture of the prep sink counter nearest the hand sink.
Recommendation: Ensure multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams greater than 0.8 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections.

Site: Food Service General-Glasswash Dish Machines
Violation: Rack-type glasswash machines located in the Lido dishwash, Tamarind dishwash, and deck 2 Main Galley dishwash were out of order. Soiled glassware in these areas were sent to nearby working machines.
Recommendation: Maintain warewashing equipment in good repair and proper adjustment, including: (1) warewashing equipment is maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; (2) water pressure and water temperature-measuring devices are maintained in good repair and accurate within the intended range of use.

Site: Food Service General-Ice Machines
Violation: An accumulation of sand-like granules were observed along the length of the plastic water return trays below the ice cubers in multiple deck mounted ice machines. This was observed in the ice machines at the following locations: Lido Galley corridor, Tamarind Galley, both ice machines located on deck 3 Main Galley Aft, and the Explorations Cafe Pantry. A heavier amount of sand-like granules were observed in the Lido Galley corridor unit then any of the others.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Pantry-Housekeeping Pantries/Ice Stations
Violation: In pantry P.1.2.01, there was a pink colored material on the side covers of the cuber panel of the ice machine and sand-like granules in the water bath under the cuber. In pantry P.8.4.02, there was a heavier accumulation of the same sand-like granules in the water bath under the cuber of the ice machine. These machines were last cleaned in October 2011.
Recommendation: Ensure food contact surfaces of equipment and utensils are clean to sight and touch.

Site: Other-Concierge Lounge
Violation: Dust and coffee debris of more than a day's accumulation was observed underneath both of the counter mounted coffee machines. This was corrected during the inspection.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Site: Pantry-Housekeeping Pantries/Ice Station P.8.2.01
Violation: Clean storage racks with clean glasses were stored on an upright trolley under a soiled tray with soiled plates, napkins, ice bucket liners, and knives.
Recommendation: Store cleaned equipment and utensils, laundered linens, and single-service and single-use articles: (1) in a clean, dry location; (2) in a location where they are not exposed to splash, dust, or other contamination; (3) at least 150 millimeters (6 inches) above the deck.

Site: Other-Concierge Lounge
Violation: The deck drain for the coffee machines located inside the pantry technical compartment was full of mold and coffee debris. The amount of mold and debris observed indicated that this area is rarely cleaned. This area was cleaned during the inspection.
Recommendation: Ensure decks are cleaned as often as necessary.

Site: Integrated Pest Management-
Violation: The integrated pest management log did not include follow-up inspection results for the inspections when pests were found.
Recommendation: When pests are noted during an inspection, ensure the log includes action taken and follow-up inspection results.

Site: Housekeeping-Outbreak Prevention and Response Plan (OPRP)
Violation: The OPRP did not include procedures to protect the passengers from exposure to disinfectants to minimize respiratory and dermal exposures.
Recommendation: Ensure each vessel has a written OPRP that details standard procedures and policies to specifically address AGE onboard. Include the following in the written OPRP: Procedures to protect the passengers from exposure to disinfectants, if not already included in the vessel?s safety management system. Ensure this protection includes: (a) Material safety data sheets (MSDSs); (b) Personal protective equipment for crew; (c) Health and safety procedures to minimize respiratory and dermal exposures to both passengers and crew.

Site: Housekeeping-Deck 2 Midship - Public Toilet Room
Violation: The passenger accessible toilet room did not have a sign advising users to wash hands. This was corrected.
Recommendation: Provide the passenger and crew public toilets with a handwashing station that includes: (1) Hot and cold running water; (2) Soap; (3) A method to dry hands; (4) A sign advising users to wash hands.

Site: Children Area-
Violation: Some toys in the children's area were not being properly cleaned. A toy pirate ship, child-sized kitchen, and a seating area were dusty with more than a day's accumulation of dust. There was also a toy spoon on the range top of the child-sized kitchen that had not been cleaned and sanitized with all the other toys.
Recommendation: Ensure surfaces of tables, chairs, and other furnishings that children touch with their hands are cleanable.

Site: Children Area-
Violation: Some toys in the children's area were not being properly cleaned. A toy pirate ship, child-sized kitchen, and a seating area were dusty with more than a day's accumulation of dust. There was also a toy spoon on the range top of the child-sized kitchen that had not been cleaned and sanitized with all the other toys.
Recommendation: Ensure toys used in the child-activity center are maintained in a clean condition.
