Before you leave your old school make sure you have these important documents to ensure that you have a smooth transition to Brewster Middle School!
Required Documents
- Social Security Card
- Original Birth Certificate
- Shot Record
- Copy of your last report card (with proof of promotion)
- Housing Assignment letter or 90 day housing letter
- Copy of Orders
- Stepparent information (if applicable)
These items will be very helpful in assigning a schedule that meets your child's needs.
- Copy of Individualized Education Plan (IEP), 504, or Gifted Education Documents
- Unofficial copy of school transcripts
- Title of textbooks used at previous school
- Course descriptions for classes at previous school
- Copy of most recent standardized test scores
- Copy of most recent ID Card
