Welcome to the Physical Exams Section. Our goal is to assist the Active Duty Soldier, Sailor, Marine and Airman in achieving medical readiness. Our hard working, dedicated team of providers and support staff understand the importance of physical exams not only for readiness, but also for promotion, assignments and health surveillance. This site contains copies of every physical exam packet with detailed instructions. You can click on the links for each exam and print a copy, or report to the ADC to pick up a packet.
Some important information regarding all physicals
· All exams are a two part process:
· Part 1 is completed on a walk-in basis during the following times:
OPTOMETRY: MON-FRI 0700-1000HRS
Contacts must be removed 24hrs prior; ensure that you bring your glasses.
LABORATORY: MON-FRI 0600-1600HRS
Fasting is required before going to Lab. This means no food or drink during this time. That also includes gum, coffee, candy, sports drinks etc; however prescribed medication and water is ok
HEARING: MON-FRI 0700-1140HRS, 1300-1540HRS
The Hearing booth is located in the Active Duty Clinic
· All Retirement physicals requires a chest x-ray; contact Physical Exams to put in an order.
· You must bring your packet with Part 1 completed to the ADC to schedule your Part 2 appointment with a Provider.
· Copies of any permanent and temporary profiles over 30 days must also be submitted with Part 1
· Please allow three weeks for specialty physicals to be completed with all appropriate signatures.
