Accident/Incident After-Action Review (AAR)

SUBJECT:

  1. Purpose: Encourag safe vehicle operations by providing unit personnel with lessons learned from fellow Soldiers' POV accidents and DUI/DWI violations.
  2. Facts: Discuss with Soldiers the magnitude of fatalities within the unit, and the devastation of the families could be the one thing that brings them home safely.
    1. Ensure POV accident reports are accurate and complete and include appropriate/quality recommendations.
    2. Discourage unsafe vehicle operations by requiring individuals (other than a driver who is suspected of a crime, e.g., DUI/DWI, speeding, etc.) to describe the incident and lessons learned in front of his/her peers.
    3. Provide a description of the incident and the circumstances surrounding the incident, the results, consequences, lessons learned/countermeasures.
    4. Tailor target audience for AAR based on severity of accident and rank of individual(s) involved.  Different severity/rank of individual(s) involved, calls for different level of briefing.
      1. Soldier killed/seriously injured - BN formation
      2. Soldier injured but not seriously - CO formation
      3. Officer convicted of DUI/DWI - BDE OPD
      4. NCO convicted of DUI/DWI - BDE NCOPD
      5. Soldier convicted of DUI/DWI - CO formation
    5. Brigade or higher review accident investigation reports of fatal accidents or accidents involving DUI.  Review to ensure accuracy, completeness, and appropriateness/quality of recommendations.
