G1 Telephone Service Request Form

Do not use these forms for telephone repair issues.  If your existing phone is not working properly, you must complete a 119 request online or by telephone. Only use these forms for requesting new services, e.g., relocating existing phone lines, installing new phone lines, etc.  If you have any questions, send an email to the G1 TCO. Select the type of service you need from the drop-down list below.  If you are unsure, contact the G1 TCO or select "Other" from the drop-down list.

If you are requesting a phone relocation and it involves swapping the location with another line, please include the information for the other phone in the Description of Work section.  Similarily, if you are requesting the establishment of a call group (allows a group of phones to pick up each other's lines), then list all phones to be included in the call group, with their respective room numbers, in the Description of Work. Please fill in all requested information.

NOTE: Use "Keep Current" for DSN/COMM Access type if you are not requesting upgraded capabilities (modification). Before the technicians will install a digital phone line, you MUST have a digital handset in your possesion.

IMPORTANT: Normal turnaround time for phone work requests is 2-3 weeks, but it can take even longer! The G1 TCO submits your request but is not responsible for performing the work. If you are dissatisfied with the telephone repair/service being provided please fill out an ICE Comment Card.
