Fort Jackson Elementary Schools Registration Information

Required Documents to Enroll

Prior to enrollment, students must meet the DoDEA specified immunization requirements listed below. Students will not be authorized to attend school until 100% immunization compliance is achieved. A South Carolina Certificate of Immunization is required.

DTP: Four (4) doses. At least one dose must be administered after the 4th birthday.
Hib: Two (2) to Four (4) doses. Hib is not routinely given to children after 5 years of age.
Hep A: Two (2) doses
MMR: Two (2) doses
Hep B: Three doses
Polio: Three (3) doses. At least one dose must be administered after the 4th birthday.
Varicella: One (1) dose after 12 months of age.

For immunization and SC Certificate of Immunization: Moncrief Army Hospital Immunization Clinic, 6th floor.
Clinic hours: Monday - Friday 7:30AM -3:30PM

Withdrawal Notification

If students are withdrawing from Fort Jackson Schools, parents are to notify the school secretary at 782-1772 five (5) days in advance.

How to Obtain Medical Forms at Moncrief Army Community Hospital

Immunization and record reviews are available Monday-Friday from 7:30 AM to 3:30 PM.
The immunization clinic is located on the 6th floor.
The clinic's telephone number is 803-751-2434.
Dental screenings are not needed, nor are they available on post.
