Using the form SSA-1699 to update the Appointed Representative Services and direct payment information

How do I as an attorney or non-attorney eligible for direct payment change my payment, notice, or tax address information?


To change or update your Appointed Representative Services and direct payment information, you must provide a Form SSA-1699 to Social Security.  You also can use that form to report changes to your address and your employer identification number (EIN) business affiliation.

For example, if you begin working for a firm and want to link clients with the firm's EIN using the Form SSA-1695 for tax purposes, submit a Form SSA-1699 including the firm's EIN.  When you stop working for a firm and no longer want to have your clients linked with the firms EIN, submit a letter explaining that you no longer want to be affiliated with the specific firm and submit a Form SSA-1699 without the firm's EIN information.
