4. Location on MCAF -- include building number and/or area of the MCAF to be visited
5. Point of contact (i.e. sponsor/employee currently working on the MCAF) -- include phone number
6. Start date aboard the MCAF (format: MM/DD/YYYY)
7. Estimated end date aboard the MCAF (format: MM/DD/YYYY)
8. Additional information and/or special instructions (i.e. entry point, time aboard, etc.)
