Subject: Federal Employees Health Benefits Program: Standard Form 2811 is no longer available

Several years ago, we made the use of Standard Form 2811, Transmittal and Summary Report to Carrier, optional. Since then, most of you have stopped using this form. We have decided to discontinue the availability of the form because of its low usage. We are revising the FEHB Handbook to remove any references to the SF 2811.
