Release of Protected Health Information Office (RoPHI)

Monday – Friday, 0730 - 1630 hours.

 

Areas of Responsibility:

The Release of Protected Health  Information (RoPHI) Office release
medical documentation for all eligible beneficiaries and also processes all third party requests. If you are currently a beneficiary, a copy of your record can be requested from your servicing clinic. Exiting the Service (ETS) or Retiring Service Members are eligible to receive a one copy of all Medical Records, to include your family members. If you are requesting a copy of your Medical Records for Insurance Company, you are only entitled to one copy. If the Insurance Company requests copies of your Medical Records, they must have a written authorization from the beneficiary.

 

                                                                                                                        

To request records:

Patient should mail a DD form 2870 completely fill and signed by patient or legal guardian. Other medical facilities or agencies requesting medical information for a patient may use other type of documentation as authorize by law.

Earlier period Medical Records:

Medical records are routinely retired (transfer) to the Government Archives various locations. Please contact your servicing medical record to confirm the status of your records. Below is a web site where you will find information on how to retrieve a copy of those records from the Archives.

Emergency request:

Emergency requests may be sent to our fax number at 910-907-8914. The only acceptable request would be those received from an Emergency Department where the patient is currently being seen or Inpatient facilities where the patient is currently admitted. All other requests received in our fax will not be processed.
