WORK
ORDERS (DA-form 4283)
If
you need a renovation project, new work, expanded self-help project or
proprietary approval please submit a Work Order. A work request is work that is
beyond the scope of work of a service order. The customer service
representative can tell you if the work you are requesting requires a DA Form
4283.
You can either submit the form in our office in Building 1059 at Wiesbaden Army Airfield or use form below to send us your work order request via email or fax.
A Signature Care is required to submit a DA Form 4283. Please verify who the authorized cardholder is.
Once
you have completed the DA 4283 please send it to the customer service center
