The Active Duty Clinic’s (ADC) goal is to be Kenner's premier health care clinic providing patient- centered primary care to our active duty population.  The Active Duty Clinic consists of two PCM teams, Team Honor and Team Valor.  Each PCMH team consists of a provider, nurses, and medics.  They are supplemented by Behavioral Health, Pharmacists and Nurse Case Managers, which help assist with your continuity of care.  One of the goals of the Patient-Centered Medical Home model (PCMH) is to ensure you are provided appointments with your Provider Care Manager (PCM) and if not possible, an appointment with another provider on your team.

The ADC offers a full range of primary care services to include minor procedures and the evaluation and management of acute and chronic injuries and illnesses.  The ADC also provides services to meet the unique needs of active duty personnel, such as specialty physicals and referrals, Periodic Health Assessments (PHAs), evaluations for overweight profiles and Fit-for-Duty and Medical Boards.  The ADC also serves non-enrolled active-duty Soldiers on a space available basis. 

To Schedule Appointments:  Personnel may schedule appointments by calling 1-866-LEE-KAHC (866-533-5242) or by self-appointing at www.TRICAREONLINE.com.  The majority of appointments are reserved for acute minor illnesses and injuries.  These appointments must be booked within 24 hours of the appointment time.  The appointment line is open Monday through Friday from 6:00 a.m. to 7:00 p.m.  Routine appointments for non-acute conditions (such as hypertension, elevated cholesterol, allergies, etc.,) may be booked up to 28 days in advance based on availability.
